Student Enrollment

To enroll your student in the UCC, please fill out the Enrollment Survey linked below. As part of the Enrollment Survey, you will be asked to upload the following documents:
 

• Waiver & Release

• W-9

• Emergency Contact Form

• Data Sharing Consent Form

• Child Heath Report

 

These forms can be uploaded directly within the Enrollment Survey or can be emailed to UCC Assistant Director Johannah Bennett at johannah@temple.edu

PLEASE NOTE: Your student’s spot in the program is not guaranteed until the Enrollment Survey and all the required paperwork above is submitted. You will receive an enrollment letter once your child’s spot has been secured.
